Today Gujarat government has formed a Committee to issue certificate of death due to COVID19.
Claimants of People who died of Covid-19 in hospital should fill up Form-4 and others to fill up Form-4A.
Doctors who attended the patient can inform about cause of death. Civic authority will issue death certificate after verification of cause of death of the patient. Forms can be filled up from 15th November.
Earlier those claimants got death certificate without cause of death can now get death certificate with cause of death mentioned in it.
Certificate can be obtained from Urban and rural areas from concerned authorities.
